* Some Medicare carriers will NOT accept the providers application until the effective date is within the next 30 days.
* The Medicare enrollment applications were recently updated. Using the wrong version will cause your application to be denied.
The Top 5 reasons Medicare Provider Enrollment applications are returned:
1. The legal name listed on the top line of the IRS W-9 form is not identical to the legal name on pages 7.1 and 11.1 of the completed application.
2. The Medicare Provider Information Form has not been completed for each practitioner, individual, or group associated with the application.
3. The Provider Agreement has not been completed for each practitioner, individual, or group associated with the application.
4. A photocopy of the provider's current license has not been provided. The license must not expire within 30 days of receipt of the completed application.
5. Page 9.4 has not been completed correctly. Page 9-4 requires information and certification by a Notary Public.
